Ten fields, about four minutes. You do not need documents to file this, and you do not need to know whether it amounts to a legal claim - that is not your job to work out.

Reports are reviewed by licensed counsel. Reporting is free, it does not file a lawsuit, and it does not make anyone your lawyer. Most reports do not become cases.

"Spring 2022" is a real answer. An honest approximation beats a precise guess.

Roughly what did it cost you?

Your best estimate in dollars. Leave blank if you have no idea.

What was the charge called?

The exact wording from the statement or invoice, if you have it.

What happened?

In your own words. What you were told, what you were charged, what you did about it, and what they said.

What records do you still have?

Statements, receipts, emails, screenshots, chat transcripts, order history. Just list what exists - do not upload anything yet.

Do not include a Social Security number, a full card or bank account number, or a password. We never need them and we do not want them on file. The last four digits of a card are plenty.

If a case moved forward, would you be open to being a named plaintiff?

The named person is in a public court filing, would likely be questioned under oath, and would hand over their records. A case needs only two to five of them. "No" is a completely reasonable answer and does not affect your report.
