What is claim form?
A claim form is the document a class member submits to a settlement administrator to receive a payment from a class action settlement, typically requiring identifying information, proof of purchase or harm, and a signature under penalty of perjury. Submission deadlines and required documentation are set by the court-approved settlement and vary case by case.
Some settlements pay every verified class member automatically with no form required, while others — especially where purchase amounts vary — require a claim form so the administrator can calculate an individual payment. Forms are usually available on the official settlement website named in the notice, not through a general search, since look-alike sites and scam claim forms are common. Supporting documentation requirements range from none (self-certification of a small purchase) to receipts or account records for larger claims.
